COACH-CLIENT RELATIONSHIP

A. The Coach acknowledges that the Client is the expert in navigating their own life, based on their personal preferences and experiences.

B. The Client is solely responsible for decisions, actions, choices, lifestyle changes, and results throughout the Coaching Program.

C. The Client acknowledges that the Coach acts as a facilitative partner and that the Coach’s primary role is not that of a content expert or educator or to diagnose, advise, or instruct the Client on what to do.

D. The Client agrees to communicate honestly and dedicate their time and energy to participating fully in the Program.

SESSION AND CANCELLATION POLICY Each session will end 30 minutes after the scheduled start time. If the Client needs to cancel or reschedule a session, the Client must do so 24 hours in advance; otherwise, Client will forfeit that appointment and will not have an opportunity to reschedule it. In the event of Client’s absence or withdrawal the Client will remain responsible for the pro-rate share of the delivered program. The Coach reserves the right to cancel the Program; Client is responsible for the pro-rate share of coaching services received.

DISCLAIMERS The Client understands that the role of the Coach is not to prescribe or assess specific meal plans; provide healthcare, medical, nutrition, nor therapy services; nor diagnose, treat, or cure any disease, condition, or other physical or mental ailment of the human body. The Client understands that the Coach is not acting in the capacity of a doctor, dietitian, nutritionist, psychologist, psychotherapist, or other licensed or registered professional, and that the Coaching Program is not intended to replace services performed by these professionals.

RELEASE OF CLAIMS The Client acknowledges that they take full responsibility for their life and well-being and all decisions made during and after the Program. The Client expressly assumes the risks of the Program, including the risks inherent in making dietary and/or lifestyle changes. The Client releases the Coach from any and all liability, damages, causes of action, allegations, suits, sums of money, claims and demands whatsoever, in law or equity, which the Client ever had, now has, or will have in the future against the Coach, arising from the Client’s past or future participation in, or otherwise with respect to, the Program.

DISCLOSURE of possible third-party compensation from purchase of product(s) that may be shared during the coaching program. The purchase of any third-party product is voluntary and not a condition of health coaching services, nor intended to diagnose, treat, or cure.

CONFIDENTIALITY AND HIPAA COMPLIANCE All sessions are held on Zoom Telehealth; any and all PHI (personal health information) exchange with the client will be shared through the end-to-end encrypted Zoom Telehealth Platform only. Client confidentiality and privacy are highly regarded and HIPAA compliant. Please see the Privacy Policy for detailed information.
